To complete your appointment, you must bring valid government-issued photo identification. Without physical ID, the Commissioner will be unable to proceed, and your appointment will need to be rescheduled.

Please note that only original photo ID is accepted — copies, photos, or digital versions (on a phone or other device) cannot be used as verification.

If your ID has been lost or stolen, you’ll need to visit a local registry office to receive guidance on replacing it. Unfortunately, we are not authorized to process appointments without valid identification.
